New York Monthly Food Inmate Package Program
 
In Accordance with NYDOCCS Directive #4911, incarcerated individuals of NYDOCCS may receive three (3) packages a month containing Food. The combined weight of these packages may not exceed 40lbs. (640 oz.) monthly. There is a $20 product minimum per order, this minimum does not include shipping or sales tax. Any orders received under this $20 product minimum will be canceled and refunded to the customer. Certain items in this catalog may be restricted at your facility and not available for purchase. Please check with you package room for a complete list of approved items at your facility. Package privileges are subject to change without notice. Please ensure your loved one is eligible to receive a package prior to placing your order. Prices in this catalog are good for 6 months, July 1, 2022 – January 31, 2023. Please use the “Food Package Order Form” to place your order.
 
TV Facilities: Attica Correctional Facility, Clinton Correctional Facility Main/ Annex, Elmira Correctional Facility, Great Meadow Correctional Facility, Five Points Correctional Facility, Shawangunk Correctional Facility, Upstate Correctional Facility and Wende Correctional Facility.
*Incarcerated Individuals located at “TV Facilities” are eligible to order three (3) monthly packages containing food items. The combined weight of these packages may not exceed 40lbs. (640 oz.) monthly.
 
PRODUCT: Union Supply Direct attempts to show and describe merchandise and all related information as accurately as possible. However, Union Supply Direct does not warrant that product descriptions or other content of our catalog, order form or website is accurate, complete, reliable, current, or error-free. In addition, manufacturers may make changes in product design, color, package size and style at any time. Union Supply Direct reserves the right to update or correct item information and to change prices and offers at any time. If a product offered by Union Supply Direct is not as described, your remedy is as follows: (1) immediately return it, unused, upon receipt while complying with your facility’s rules and regulations. (2) If an item is unavailable for any reason, we will obtain approval by the customer to substitute with an item of equal or greater value. We regret any of these changes and make every effort to ship the proper product that you request.
Free items are only available while supplies last. If a selected Free item is out-of-stock or discontinued, the company will substitute the selected Free item with another item of comparable value, or cancel the Fee item if no comparable substitute is available.
 
PAYMENT METHOD: Union Supply Direct accepts the following forms of payment: credit card, personal check or money order (must be domestic, printed in the U.S.) or cash payment through ACE Cash Express. We accept all major credit and debit cards with the following logos: Visa, MasterCard, and Discover. There is no surcharge for using your credit card to make purchases. Please be sure to provide your exact card holder name, billing address, CVV code and telephone number that your credit card issuer bank has on file for you. Your credit card will be billed upon placement of your order.

Credit* When order ships There will be a hold on your credit card for the amount of the order. The hold on your card will be removed and your card will be charged at the time the package ships.
Debit* When order ships There will be a hold on your debit card for the amount of the order. A positive balance must be kept in the account until the package ships at which time your card will be charged.
Prepaid* When order is taken Funds are deducted from your card and placed on hold. A positive balance must be kept in the account until the package ships at which time your card will be charged.  Additional time will be needed to process refunds.

Be sure to keep the receipt provided by ACE Cash Express, for record of your payment. Allow 2 business days for payment to be applied to the order. Full payment must be received by July 31, 2021 or it will be cancelled. All refunds will be processed by Union Supply Direct and sent to the customer on file for the order. SALES TAX: Sales Tax is applied to tangible personal property, candy, cosmetics and toiletries (Tax Code B – Taxable Items). All Clothing and Footwear (under $100) are eligible for a reduced sales tax (Tax Code C-Reduced Taxable Items). Food (except candy) is not taxable (Tax Code A – Non Taxable Items). Please see next page for applicable sales tax per facility.
 
SHIPPING: All shipped orders will be charged $8.95 (Non-Refundable). Cancelled orders will not be charged.
 
RETURNS & REFUNDS: Union Supply Direct is not responsible for refused items. Please ensure that the items you ordered are approved at your facility. If an items is not allowed at your facility, please be aware that Union Supply Groups is not responsible for return shipping of these items. Used or abused items cannot be returned, exchanged or refunded. Please check all your items upon receipt. If any of these items are found to be missing, defective or damaged in shipping, please notify a facility staff inventorying your package immediately. Damaged or defective food and hygiene items will be refunded. Non-food items may be returned for a refund or replacement. If the original payment was made by credit or debit card, approved refunds will be credited back to the card used in the original purchase, including prepaid credit and debit cards. All merchandise that was ordered in error, for any reason, can be exchanged for $6.95. All exchanges or returns must be done within 30 days of purchase. 
  
Restocking Fee: If a package or item(s) is returned by you or your facility for any reason other than because the item(s) are damaged or the wrong item(s) were sent, a restocking fee of 10% will be deducted from your refund. Shipping Fees are non-refundable for refused packages.

PLEASE NOTE: All products must come back to us directly from the facility. If for any reason you decide to send the package to a friend or family member, instead of back to Union Supply, then that sale is final and no exchanges or refunds will be issued. For security purposes, we cannot accept any products or packages that have left the facility. All original boxes and packaging material are required on any returned goods.
*Swintec typewriters must be returned to Swintec Corporation for repair once engraved if warranty service is required or for any exchanges. See address below.

New York Personal Property Inmate Program

Family, Friends and Incarcerated Individuals are responsible for ensuring the items they order are allowed at their facility and the Incarcerated Individual does not exceed the allowable possession limits. No Incarcerated Individual shall possess property (combined State and personal property), including legal material, in excess of that which can be placed in for (4) standard Departmental draft bags. There is a $20 product minimum per order, this minimum does not include shipping or sales tax. Any orders received under this $20 product minimum will be canceled and refunded to the customer. Certain items in this catalog may be restricted at your facility and not available for purchase. Please check with you package room for a complete list of approved items at your facility.
